Colsolidation of previous Evaluations
Consolidating previous evaluation data and conducting a thorough record review is a crucial step to create well-organized assessment reports. These reports serve as a foundation for treatment planning and recommendations, helping to ensure that clients receive appropriate and effective care.
This process involves gathering, synthesizing, and analyzing all available information about the client's psychological history, assessment results, and relevant records to provide a comprehensive and well-informed assessment report.
Cost: $350 for the first 150 pages of record review ($0.35 per additional pg.)
Includes:
Data Collection: Dr. Moreland starts by collecting all relevant evaluation data and records. Including previous psychological assessments, medical records, educational records, therapy notes, and any other pertinent documents.
Datasheet: All of the data (actual scores and/or brief summaries of medical/mental health progress notes) is collected and organized into a datasheet document that can be used to provide current and future treatment providers.
Optional Add on Services include:
Report Writing ($500): Dr. Moreland can prepare a detailed assessment report that synthesizes and integrates the data reviewed. This report would include a summary of the client's history, assessment results, diagnosis (if applicable), treatment recommendations, and any other relevant information. The report is organized in a clear and structured format.
Consultation ($180/per hour billed in 30-minute increments): Dr. Moreland can consult with other professionals, such as psychiatrists, social workers, or counselors, to ensure a holistic and well-informed approach to treatment planning.
Feedback and Discussion with client and/or caregivers ($180 for 45-60 minute session): Dr. Moreland is available to discuss the findings and recommendations. This allows for collaboration and ensures that the client's preferences and goals are considered in the treatment plan.
